Almost never a break-in. It is people inside the building who should not be, and the usual way they got there was walking in behind somebody with a pass. Most of it is controlled at the point of entry, by a person rather than a credential.

This needs to be written into the post orders before the first shift, and it is the single most common gap we find. Name who can authorise access per tenancy, how the officer verifies that person at 11pm, and what happens when nobody answers. Without it, the officer is left choosing between refusing a legitimate contractor and admitting someone on an unverified say-so, and both of those become your problem the next morning.
